Welcome!  As a ministry to the Palm Beach Atlantic University community, our mission is to promote physical, mental, and spiritual health and well-being.  Our desire is to provide all wellness services and programs from a Christ-centered perspective, enabling each person to fulfill academic, personal and spiritual goals. Counseling & Wellness provide free mental health services. We also offer a wide range of health programming to the university community including educational programs and regular blood drives in cooperation with area community blood centers.

 Domestic Students - Health Insurance Information 

The university does not offer a school-sponsored insurance plan. However, it is strongly recommended that students obtain coverage while enrolled at PBA, as the university acknowledges the significance of healthcare protection for its community. Below is a list of health insurance options tailored to specific student groups, which can aid students in making an informed decision. International Students - Health Insurance Information 

The university does not offer health insurance. However, the United Healthcare student health insurance plan is available as a resource for F1 visa international students. PBA is not affiliated with United Healthcare. We strongly advise students to review the policies of the student health insurance plan carefully before purchasing coverage.

Fall semester: Health forms are due on August 1st. 
Spring semester: Health forms are due on December 15th.

Required Forms

Graduate, Evening, and Part-time Students

  • Complete the electronic TB Risk Screening Form.

Full-time Undergraduate and Additional Bachelor

  • Click on Forms to access all the required forms.

  • Immunization History Form - All Full-time Day Undergraduate students attending PBA's Main Campus are required to have MMR 1 & 2.  Students living on campus also need to have Hepatitis B 1, 2, & 3 and Meningococcal ACWY (not MenB vaccine). Print the Immunization Form and have a doctor complete it. Visit the MedProctor to access the health forms. 
  • Once the health forms are completed, uploaded supporting documentation into the MedProctor.
  • Physical Exam Form - All Full-time Day Undergraduate students attending PBA's Main Campus must have a doctor complete a physical within 12 months prior to your start date.

Pharmacy, Nursing, and Athletic Training students may be required to submit additional requirements to their departments.

 

 

Required Forms
No additional forms are needed; however, we do recommend that you submit any updated records (i.e. physicals and immunizations from other medical facilities). If your health information (i.e. allergies, surgeries, etc.) or contact information has changed, please complete a new Health Information Form.
