The Preceptor Plan Application must be completed prior to the start of a course that requires clinical hours.  The form should be completed and returned to the program coordinator and to the course professor on the first day of classes. The form must be uploaded to the Dropbox in your course, as well as the Medatrax program, to maintain records for your certification upon completion of the program.
